If you feel that your patient may benefit from the use of medical Cannabis and are unsure of how to proceed, you can simply refer them to our clinic where we can handle the whole process for you and your patient. To refer a patient, simply download our referral form below or contact us directly. If you have multiple patients we would be happy to send you referral pads and supporting materials as needed.
